(a)-(c), (f) This Statement is filed by The Oregon Community Foundation (the "Reporting Person"), whose business address is 1221 SW Yamhill St. Ste. 100, Portland, OR 97205. The Reporting Person is an Oregon nonprofit corporation and is recognized by the IRS as a 501(c)(3) tax-exempt corporation. (d)-(e) During the last five years, the Reporting Person has not been (i) convicted in a criminal proceeding (excluding traffic violations or similar misdemeanors) or (ii) a party to a civil proceeding of a judicial or administrative body of competent jurisdiction and as a result of such proceeding was or is subject to a judgment, decree or final order enjoining future violations of, or prohibiting or mandating activities subject to, federal or state securities laws or finding any violation with respect to such laws.
